   • Cost $25
   • Fee and prescription required before shipping

2. CPAP machine- prescription required

• Set up includes: machine, new tubing, filter, carrying case and data card
• Apply online or print out and fax application
• Patient or physician representative may apply
• $100 fee

Patient application

• CPAP only
• Mask size (S, M, L)
• Mask style (Full face, nasal, pillows)
• Requires prescription (send with application, email or fax)
• $100 program fee (money order, credit/debit, money order, certified check)

Physician Application:   Same as patient application with the additional option of ordering APAP

   • Good option for patients looking for CPAP masks, tubing, filters, masks, and other CPAP accessories  (other than their DME)
   • Prescription is required for machine, mask or humidifier
   • Allows patients to purchase a mask, try it for 30 days and then return it if they do not like it or it does not work for them. They must select the “mask insurance” option when placing online order (mask insurance is free for most masks). With selection of this insurance, a ground return shipping label is sent with the patient order.
   • Company does not bill insurance but does include an example of a Health Insurance Claim that is populated with order specific information and billing codes (It will be included with the shipping confirmation email).
   • Patient may want to check with their insurance company for out of network reimbursement rates before placing an order.
